in order to execute services at my property with respect to;

I hereby confirm that I understand that I am the employer of the specific person and take responsibility for him/her from entering the Estate.

I hereby indemnify the Silver Lakes Homeowners Association and Board of Directors against any liability related to the above mentioned person and all people he/she comes in contact with.

I undertake to monitor the person on a daily basis with respect to symptoms of the COVID-19 virus and undertake to inform the Silver Lakes Homeowners Association immediately upon the specific person showing any symptoms of the COVID-19 virus.

The Silver Lakes Homeowners Association retain the right to refuse the abovementioned employee entry in to the Estate should he or she not adhere to the lockdown regulations or show any symptoms of the COVID-19 virus.
